Family care and dementia support services

The Family Caregiver Support Program champions caregivers, provides information and links to programs and services to support your caregiving efforts, and provides powerful tools and tips for a better caregiving experience. It is for all caregivers living on Martha’s Vineyard or caring for someone living on Martha’s Vineyard.

Services

One-on-one sessions with a trained clinician to help caregivers navigate resources and manage care for their loved ones and themselves. Services may include:

  • memory screenings
  • care advice
  • habilitation therapy
  • information and recommendations
  • Facilitate family reunions
  • Education and support for Alzheimer’s/dementia

care advice

We can help you navigate the public and private care system to find the best plan for your needs. The consultation can take place at your home, in our office or by telephone. We can meet with the caregivers individually or as a family.

habilitation therapy

Post-doctoral therapy is a non-medical, interpersonal approach to caring for people with memory loss. The habilitation strengthens the existing skills to improve and maintain functional independence. Learn how you can simplify tasks and help your loved one become more fully involved in their own care and actively participate in life. The habilitation approach helps the caregiver manage behaviors, reduce caregiver stress, and can help facilitate a more rewarding caregiving experience.

Self-help group for dementia caregivers

The Dementia Carer Support Group provides a confidential and supportive atmosphere in which carers can come together to find encouragement, comfort and advice from others who share similar experiences and concerns. For free.

dementia friends

Dementia Friends USA is part of a global movement changing the way people think, act and talk about dementia. By helping everyone in a community understand what dementia is and how it affects people, each of us can make a difference for people affected by dementia.

By hosting a Dementia Friends information session for your employees, you are helping the community better serve our neighbors. This free, hour-long training session can be added to a staff meeting or scheduled as a special event.

Please contact Mary Holmes, Dementia Friends Champion, at [email protected] or 508-939-9440 to schedule a free session.
